Please do not fill out your reports in all capital letters.

Service With Style will only reimburse up to 15% of your bill for your tip.

Disable any pop up blockers while typing your report.  

Please be sure to SAVE your work every few minutes and prior to submitting your report or you will be timed out and lose all the information that you have typed.

"Effective Immediately" Reports are due within 24 hours of your scheduled observation.

It is very important to pay close attention to your visit details when scheduling and filling out your report.

If you do not submit your itemized receipt within 24 hours of your scheduled observation  you will be fined.
